It's certainly possible to be depressed but not know it. I had a coworker who fell into poor health for seemingly no reason. His job performance waned, and he was often out sick. More than a year went by before he was diagnosed with anxiety syndrome and acute depression. He was sort of a bible thumping gun-nut though so I didn't much care for him, I'm guessing he was pretty big into the whole teabagger thing and probably a racist.
