Sexual differentiation and development are governed by factors other than simply whether a person is XX or XY. The development of male physical characteristics is dependent on the presence of male hormones.

If a fetus possessing XY genes does not have the proper release of hormones, it will not develop male characteristics. What you are talking about is post-natal rather than prenatal sexual reassignment. There are reports that it does happen. A child appearing to be female will experience a delayed release of male hormones and begin developing male sexual attributes after birth. This happens in nature because the XY genetic material is present but is delayed in coming online. Sexual surgical reassignment requires hormone therapy for someone transitioning from female to male is required because the XY genetic material isn't there, and for someone transitioning from male to female because it is and needs to be overcome. Remember though that while the male and female sexual organs are the primary source of testosterone and estrogen, there are elements of each in all humans.
