Can someone tell me how the fuck this can happen?

Telemarketer spoofing your number. Happens all the time.

Sign up for Obama's free telemarketing blocking service

https://www.nomorobo.com/

Works great for me. Rarely do any telemarketers get through. Phone rings once and the if it a telemarketer on the server block list it is then blocked.

Completely FREE service!

Unfortunately, Verizon doesn't support nomorobo.

This seems to be a recent "innovation". We've been getting them close to our number--same area code and nxx, just the last 4 are different. Rachel or her equivalent.

The way CLID works, the number is what's transmitted: there's a database lookup done by the local exchange carrier (your provider, or near to it). That's why occasionally you'll get OUT OF AREA for a number that normally shows up: the database dip failed for some reason, but the number still made it through.

I bet Bell and Bell Northern (later Nortel, now dead) wish they'd made CLID non-fungible. Or maybe they weren't allowed to by regulations. Either way, SS7 and DMS-100 (the two main digital switches, last time I checked--likely different names and evolutionary versions now, but those were the names when they were developed) let CLID be sent by an ACD, rather than enforcing it a la ANI (the technology used by toll-free numbers).

Of course, at the time, VoIP wasn't even a gleam in anyone's eye; not clear what, say, Skype-to-landline would send for CLID if it couldn't fake it; all zeroes?

Meanwhile, the Do-Not-Call list is, alas, a joke.
