He must've been bleeding heavily before he hit the water, otherwise I'd say they scavenged him.

I'm gonna have to go with fake. I used to have some piranha and kinda understand their behavior. Though I do think they're capable of this, I don't think they would attack someone in this way. They would start at the fingers and toes and work their way up, and only if the victim were, very still or bleeding or already dead.
They prefer smaller, easier targets as well.
less than 0% chance piranhas did that, if "that" is even an actual human. if that is in fact a human, something other than a school of piranhas removed the flesh from his face. if it was, the body would have quarter size bite holes all over the place, or at least what part was in the water.
in the wild a school of them will go after large prey in the water, like cows and what not, but they typically prefer to dine on those capybaras, or whatever theyre called.
He was in a bulldozer-type excavator that was too close to a weaker spot on the bank. It collapsed and the bulldozer toppled over into the water. He had to break a glass window to try and escape, and the broken glass plus injuries sustained in the initial fall caused a lot of bleeding, which attracted the various predatory fish. This occurred quite quickly, but unfortunately, this much damage had been done before co-workers could pull him from the water.

Thanks for the backstory. I wonder how painful that is to have fish nibble off your face....
