In the law there is a saying 'you take your victim as you find them...' Really doesnt matter if she died of a brain bleed or heart attack or internal injuries - if you caused it its manslaughter or murder and/or wrongful death (civil).

Cardiac arrest is a symptom brought on by shock and other injury to the body. Just because she died because her heart stopped does not mean she wasn't going to die and does not mean the intention nor the consequence was not murder by means of an act of terror.

The eggshell skull rule is a rule of civil law rather than criminal law. Since yelling "boo" at someone is not ordinarily a tort, it isn't a good example.
This is a criminal case and is governed by criminal laws. The rule for criminal law is if you intentionally or recklessly create a situation that poses a substantial risk to the life or health of others, you can be convicted for the injuries directly sustained as a result of your acts and for the injuries suffered as a result of the efforts of others to avoid you. The classic example of this is intentionally lighting fire to an occupied building. The arsonist can be charged for the deaths of those who dies from smoke inhalation, from those who burned to death, from those who were killed by falling debris and from those who died attempting to escape from the building. It doesn't matter that some if the victims could have easily and safely escaped if they had been in decent physical shape and had kept their wits about them. If grandma died because she couldn't move her walker fast enough or Uncle John died because he was too drunk to notice the fire until it was too late, or the triplets are trampled by others trying to escape, you are screwed.
