gah... the nerves
inside your skin is how you feel pain. if you really want to inflict pain, you remove the outer layers of skin so that you can get to raw nerves. this is why second degree burns hurt and third degree burns do not.

going into shock is a psychological reaction, not a physical one. therefore, you can inflict extremely high and prolonged amounts of pain if done properly.
if you really want someone to feel the maximum amount of physical pain, directly stimulating the area of the brain responsible for the sensation of pain would be most effective. however, this is torture, not a particular way to die.
