Is it okay to stay in the passing lane on I-66/I-495 in stop-and-go(10mph) traffic, or should you move over?

If there's only light traffic, I always get out of the passing lane, if I see
a car coming up behind me at a faster speed. It's just common courtesy.

However, what if the traffic is real heavy... stop-and-go at 10mph or so.
Should you stay in the passing lane alone with thousands of other cars...
or not?

If you're immediately (as in close) behind the car in front of you, then you are not impeding traffic. In rush hour conditions like you describe, it basically ceases to be a passing lane. If it's that bad using all 4 lanes, imagine what it would be like if everyone avoided the left lane, then there would be in effect only 3 lanes.

I agree with this.

If you are within normal safe following distance of the next car ahead of you it's ok to stay in the lane. However, if you are on 395S in the seperated HOV lanes (speed limit 65) and you drive 60mph in the left lane, you can rot in hell. It boggles my mind why people consciously choose to drive the HOV lanes, with a higher speed limit, and then they insist on driving slower than the limit.
