Coverages are set by applicable insurance policies. PPACA is not a public option. It mandates certain minimum coverages for all policies privately issued so that anti-social underinsured-wannabe fucktards and morons can have their sorry and pathetic asses dragged kicking and screaming past the point of being forced to pay their actual fair share. Coverage otherwise is something to take up with your insurance provider.
