Anyone know of a good Pain Management Doctor??

Hello all,
I'm new here and am hoping to find some information from the Fairfax locals.
Just to give you a breif description of my disabilities....

I injured my back two yrs ago at work and have been on workers comp for two yrs now. I've had two failed back surgery's. A spinal fusion at L5-S1, then due to severe pain due to the hardware, had those removed a year later, only to find out that they have caused even more damage to my joints where the hardware was placed. I now suffer from Sciatic joint pain, slight scoliosis, DDD, Narrowing of my L3-L4 disc's (which surgeon now wants to do yet another fusion there), Spinal stenosis, Neuro-stenosis & Facet joint problems.

My lifestyle has changed 100% and all the damage is irreversible. Along with a team of surgeons that I have, I am now seeking a Pain Management Doctor to help manage my daily pain. I have done 12 weeks of physical therapy with no improvement, pool therapy, massage therapy, and am now on a cocktail of 7 different medications to help control my pain. On top of all that, just this week, I had joint injections, nerve blocks and epidural blocks, all with no help, just additional pain from all the injections.

So if anyone has any recommendations of a good pain management center or doctor any suggestions would be greatful and looked into.

In all seriousness, not jokingly, you should consider smoking or in some other way consuming marijuana. I consider it a miracle drug for pain relief; I have scoliosis and it causes back pain that ruins my appetite and makes sleep frequently difficult; a few tokes and my mind goes away form the pain, and it has the added benefit of being an anti-inflammatory (the chemical (E)-BCP (beta-caryophyllene) as well as the THC itself) for the back muscle inflammation my disorder causes.

It's also great for relieving boredom, stomach pain, cramps, hangovers, and having to watch bad movies and listen to shitty music.

And oh yes: any addiction to it is minimal and quite easy to get over, far easier than quitting opiate drugs that have awful side effects and are addictive as hell, and much easier than quitting smoking tobacco.

Is there a doctor in this area that prescribes medicinal marijuana that you are aware of, or is this something that is obtained illegally? I know that for cancer patients many physicians will prescribe what you are speaking of, but i've not heard of a prescription for anything other than cancer patients.

I don't think even for cancer patients they can legally prescribe it.

However, if you really have pain, and marijuana is what works, then just be careful, buy it from someone you know and trust and don't carry it around outside your home.

There was a documentary on HBO about a year or so ago where they interviewed several people of varying ailments who obtained their marijuana illegally and were willing to take that (fairly minor) risk. They just didn't buy large quantities at any given time, and they were careful to only use it within their home and to be upstanding and law-abiding citizens in every other respect.

They had one lady who had something like MS or Cerbral Palsy who couldn't care for her children unless she smoked a half a bowl of weed first think the morning to get rid of the shakes and uncontrollable muscle spasms. She didn't smoke in front of her kids, she was discrete and wasn't using it to get high and listen to tripping music, she used it only to be able to funciton.

There is "marinol" that can be prescribed, but I've heard it actually causes nausea and barely provides any benefits.

It's up to you if it works and you are willing to take the minor risk of being messed with by the law. And if you're worried about the health affects of smoking you can make canna-butter and eat it, or get a vaporizer and inhale the THC without all the bad stuff you inhale when burning it.


You could also look into non-pharmaceutical pain remedies. I'm not sure how good any of them are. FOr example, instead of Aspirin, there's White Willow Bark. Not that this would be much help for chronic pain like you describe, but I just mention it because there are probably other natural substances that have been synthesized in a lab that might be less harmful than the synthetic forms.

oh yeah, I forgot to add that ibuprofen actually works quite well in higher doses. Though mostly with muscle pain, not bone and joint pain, so much. 1,500 mg doses can do wonders. It's not exactly great on the liver and I wouldn't consume any alcohol while taking IB, but at least it is less harmful and I'm pretty sure non-addictive since it's just an analgesic. Of course, it wouldn't hurt to have something like oxycodone on hand for times when your pain becomes too much to bear, but I would only use it rarely, when you cannot find relief by any other method.

No matter what pharmaceutical you take, I'd take measures to protect your liver with milk thistle and alpha-lipoic acid. Most pharmaceutical drugs put a heavy load on your liver.

But I do appreciate your help and will try searching for other options. Unfortunatley the last physician I saw readily handed out any narcotic that was asked for so for two yrs I was treated with opioid therapy, which worked wonders for my pain, but I built up a tolerance level very quickly and when they decided they no longer wanted to prescribed the amounts they were, they gave me a week to find a pain management specialist.

There is a pain management practice co-located with the Virginia Spine Institute in Reston. I'd recommend them unreservedly. They also have a physical therapy practice there that outclassed the large chain practices.

You obviously don't go to their Herndon location. I've never waited more than 10 minutes during my GF's visits. She gets amazing care there. I have been present at every appt. she has had and the care they give her is the BEST ever. Perhaps you got hooked up with another doctor that overbooks patients there.

If you are interested in trying something that will work and is all natural/with no side effects. Go to mydoterra.com/essentialbodyandmind. frankincense, lavender, lemongrass, marjoram. those are the oils I would suggest trying, you would just rub them on your feet daily, and also to directly on area! I have had amazing result with the oils! Love them and never have to make dr. visits, they have some thing for every thing!Hope this helps!

My husband goes to Capital Spine and Pain. He had the epidural blocks and one back surgery,about 4 years ago, and is now going again for the blocks (taking him for second one tomorrow in a month). He wasnt happy with it last month in that it didnt help and the dr doesnt give enough meds to last him the 30 days. I think this past visit they gave him an opiod (sp?) drug in addition to vicodin. Ehhh on helping.
Now I have heard someone tell me the name on a particular dr he needs to ask to see, but the wait is very long to see her, a month at best. Wondering if maybe we need to find out who that dr is and make an appt with her. We will see what happens with tomorrows visit.

They gave me an epidural nerve block shot. It hurts like a motherfucker. Like getting struck by lighting. But the back pain subsided in a couple of days after the injection. Give it a shot (no pun intended).

Try THE BRANER CLINIC it is off of rt 50 in merrifield. they are really good there, usually about three weeks till you can get a appt, but once you do, you will be impressed. Dr Gonlalez is wonderful!!

Chronic Back Pain is really a effed up thing to live with. I have had the nerve blocks and agree they are painful, do they work? yes only for a couple days. Pain Meds can work good as long as you use them the way you are suppose too. I had surgery and it worked. so I don't need meds now. But yes there are alot of options, swimming really helps too

Capitol Sine and Pain Center, well, they will give you more than the recommended amounts of steroid like injections and painkillers and then when the injections continue not to work they will recommend you see someone else. Dr. Nguyen is Reston, at Virginia Spine Institute is nice, again will try to push treatments such as accupuncture but does prescribe medication. I saw him for over a year and then he decided to take me off the medication he had me on, and never really seemed to care otherwise what was going on. He would prescribe meds that were very expensive and then when they made me violently ill, had me flush hundreds of dollars of medication down a toliet with his assistant watching. I got a copy of my records from his office and there were pages upon pages of errors and flat out lies. I would not recommend him to anyone....I have had spine surgery, have failed fusion, DDD, insomnia, fibromyalgia along with other things and I am now just trying to find a doctor that will actually LISTEN rather than just give you a prescription and another appointment in a month. Yes I am in pain, and yes it is constant, but I would actually like to have a docot that examined me, talked to me and cared if they were helping me or not...anyone have any real suggestions? There are so many posts on this thread that are just plain rude and are obviously written by people who have never been in pan or had any medical problems and just need to grow up!

dr sam ore in woodbridge hospital and dr at mount vernon hospital i know its a drive but she put me on 100 mcgh fentanyal patches and they worked great for two day and plus its the hospital were superman went after his horse accident they are real nice and she listens cant rember her name but call mt vernon hospital and ask for a pain doc she the only one in the hospital you walk in main entrance and make a left at the first hall and her office is the first one on the left

Don't let docs fuck with you re: pain. Oxycotin is the best pain killer as it does
not cause depression, etc. Marijuana is a fine muscle relaxant.
Becoming addicted is a big "so what".

The whole purpose of pain management is let you function.

I went to Capital Spine and found the same mess. They will prescribe lots of meds for awhile then insist on trying all of thier torture treatments while holding the meds hostage. Then when the injections etc. don't work, they won't refill or write for anymore meds. They just sent me out the door to detox. I found the doctors there to be very egotistic and judgemental. I was told I didn't have pain after I was hit by a car and nearly lost my leg up to the iliac crest!!! It's just my opinion but I would stay very far away from Capital Spine and Pain.....

Op..... You sound just like me. I have had four back surgeries which also made things a lot worse and I now suffer from problems that the surgery caused. I have been going to Capital Spine and pain for three years and have been very pleased with the outcome there. They just recentley merged with The spine center. They have approx 49 physicians and 25 offices located in Va, Md and DC. I have never waited more than 10 mins for my appts. I suffer from DDD, SI Joint Dysfunction, fibromyalgia, and osteo-penia. I would try Capital Spine, as I have tried others that didn't do as well as this group. .

I hurt my back when I was teenager and struggled with crippleing back pain for many years until in my early forties I became totally disabled and couldn't do anything including sleep for more than an hour at a time. I was in misery for 9 months. My sister-in-law gave me a bottle of chondroitin glucosamine(osteo-biflex). Within 30 days of taking three extra strengths every day my pain had lessoned by 60%. After two months I was pain free and back at work. It's now been 15 years without any back trouble whatsoever. Though your back problem is probably worse than mine I'd bet that this medicine would benefit you greatly. Wish you well.

I went to Capitol spine in Mclean. My PCP referred me to Dr. Dade who does listen and treated me well. Two injections and sent me to Promotions physical therapy for strengthning. No pain today and now I know how to take care of my back. Great experience fast and to the point.

I would highly recommend Dr. Nguyen and his team over at the Pain Managment Center of Virginia (Reston). They are located with the Virginia Spine Institute so there are a ton of resources right in house (including physical therapy). They are the best in the region and I've seen some of the Redskins players in there getting treated so they must be doing a good job!

DO NOT USE VA Spine Institute. I have similar situation -- really permanently messed up back. One disc herniated, every other disc on the lumbar MRI bulging and also degeneration all over and SI Joint Malfunction -- I went to VA Spine Institute for the neurosurgeon, who I thought was top-class. He caught the SI Joint issues where it had been misdiagnosed before. He referred me to their pain management and physical therapy. Physical therapy was outstanding; I saw Rich -- awesome!!! Pain management, not so much. I saw Dr. Nguyen there for pain. He wound up ODing me on morphine (or I am sensitive or something). I couldn't sit down for more than 10 minutes without nodding off and when I woke up my ankles and feet were swollen three times normal size, like my heart had slowed down on pumping back the blood and fluids. I fell asleep leaving their office in Reston, fell asleep at the wheel and woke up after I'd crossed all 4 lanes of 28. I told him the morphine was either too strong or something and he took me off of it. Later, I had to cancel and reschedule an appointment because my son had an appointment at his school for special ed on the same day. The next appointment I tested positive for morphine (I'd been off of it for a while and on oxycodone). The only thing I could figure was the two pills were the same size, shape and color -- just the oxycodone had an M on it and I found a few pills in the bottom of my change purse that I took. I thought the M meant morphine and threw those out, when they were really what I was supposed to take & I took the others. Anyway, Dr. Nguyen found me to be an addict ("non-compliant") for "missing" an appointment and for having morphine in my system. He even kicked me from the practice where I couldn't see my neurosurgeon. And all of this got put on my permanent record. The real loss to me was loosing the neurosurgeon -- which was why I picked that practice to begin with. Major trauma!!! I used to think Dr. Subach (the neuro) was totally awesome -- but now not so sure. After all, I'd been seeing him for 2+ years and he didn't stand up for me. I was going to see an addiction person; my mother talked me out of it because she lives with me and told me it was a waste of money. It's major trauma to lose the guy you trust to cut on you though. DO NOT GO to VA Spine.

Georgetown University referred me to National Spine and Pain Management. Told me it didn't matter who I saw, they were all good. My sister has been seeing a Dr. Mayo Friedlis who ranks high on Washingtonian Top Docs and is listed in US News and World Reports. He found my sister's fractured hip that no one else had caught. I've also heard good things about the doc in Manassas.

To sum: skip VA Spine -- they over-drug you and find you non-compliant without even following the federally recommended guidelines on that issue. Go to National Spine and Pain Management.

So you had two very dangerous prescription drugs, one of which you have "a bad reaction" to, and didn't bother to keep them separate? I wouldn't treat you either -- too much risk. Some people are too stupid to help.

DO NOT FOR ANY REASON GO TO CAPITAL, Spine, and CRAP!!! Especially the one located in Woodbridge, va. The dr there is named Dr Shambaugh(?) or something like that. I have many, many back/hip related issues. All he gave me was the lowest dose imaginable of meds. Now all he wants to do to do injections into my spine and hips. Even after I did two rounds of injections (equaling 12) injections of lidocaine! Yes LIDOCAINE, the same family of numbing agent a dentist uses. This crap wears off in few hours to two days, then you're back to feeling the same or even worse. Mainly due to NOT feeling the pain you're able to inflict upon yourself. When I called and said the medications do not help at all, they made me do another appointment...not to speak to dr Shambagh, but his PA or NP. They informed me I will never be able to have higher doses of the medications I was on. The nurse would run between the patient room and to the blessed wizard of Oz to relay what was being discussed. After I demanded to speak to him directly I was instructed to leave his office and to never return. When I contacted their headquarters...they informed me, I was not allowed to be seen by any of their drs ever again. Eff them, they all can kiss my old black butt! I should just start smoking like my grandson keeps telling me to do. Hope this helps other make a decision to better yourself. These drs are only after $$$$ and what they charge my insurance company for a 10min conversation $700!!! Complete BS and a rip off!!!! All drs suck and treat us like drug addicts. If I would ever see him out in public I will take my walker and bust right in his ding ding, over and over again. Let him know what i feel like 24/7 for the past 25+yrs!

have been diagnosed with like 6 different arthritises, and have arthritis in spine

was put on fentanyl, patches were supposed to last 3 days, lucky if lasted one before withdrawal symptoms kicked in

got off of that cold turkey

have been using a tesla violet ray, a high voltage coil oscillator that uses glass bulbs with argon or neon gas in them, that i run up and down spine and over other arthritic areas

really helps if you use it daily
the american companies that made them were killed off in the fifties by the government
the chinese and russian units don't put out enough voltage to make them effective
got some old units through ebay but they both died
going to order an american made mjolnir
the only way these can be sold today are for bdsm fetish play, no medical issue units

Well the doctor I was seeing was busted, I found out when I went to go fill my script! CVS will not fill anything of his! I need to find another pain management doctor, I have MRI's to prove my back problems.

Dr. Gordon, mentioned above, gave me 3 epidural shots. The first hurt like hell. The second one didn't hurt, and the third shot again hurt miserably. I am swearing these things off. These shots shouldn't hurt that much. Do not go to him.
