FCPS wants my position on ABORTION

My daughter just brought home a "Health" class project that she said I (parent) have to participate in and the form REQUIRES my signature. There is no additional instruction, cover letter or explanation as to the use of the document or who will see it. The questions on the form are to be rated "Agree", "Disagree",or "Unsure".
the directions say: According to your moral or religious values, indicate your position.
1.Abortion should be allowed if the mothers life is in danger
2.Abortion should be allowed if a fetus is not developing
3. Abortion should be allowed in cases of incest
4. Abortion should be allowed in cases of rape
5. Abortion should be allowed for any reason
6. Abortion is murder in all cases.
7. A woman should not be forced to have a baby she does not want
8. The father of the unborn child should have the right to deny or allow the abortion
9. The father of the unborn should have the right to demand an abortion
10. Life begins at conception
11. Parents should be informed if their dau.(a minor)goes for an abortion
12. An adoptee should have access to the legal records of his her adoption in order to find the birth mather or father
13. an unmarried teenaged father has a financial obligation to support the mother and child
14. a high school age couple should marry if the girl is pregnant
15. Married couples should jointly decide whether to have children and when.
I am shocked that the FCPS (this form was approved by FCPS School Board) is expecting people to fill this out, put their name on it, and return it to the school with out explanation on its use, how it will be disseminated, why it is needed, and how privacy will be protected. This is PUBLIC School, not religious school.
