Pronoun mis-identification goes way back, and frequently has nothing to do with transgenders and transsexuals. Referring to men using female pronouns or titles was a common insult. Unfamiliar names result in the use of the wrong title.
Then there are the phone calls where the woman with the husky voice is referred to as "sir" or the man with higher tone voice as "ma'am".

There is a simple solution to all of this. We abolish the use of all male pronouns and refer to everyone regardless of gender or gender identity by female pronouns. For titles the capital "M" is to be used unless another non-gender title is applicable. Where job titles are gender specific (waiter/waitress) the use of the diminutive female form is abolished elevating all to the male title.
