You haven't said anything to indicate a crime was involved.
It looks like a straight workers comp claim.
The system is set up so you can handle the claim yourself. You don't need a lawyer, although having a lawyer may protect you in the long run, and a lawyer is particularly necessary if your claim is denied.
Generally speaking, the more serious your injury, the more seriously you should consider contacting a lawyer. Particularly if you've been disabled by the accident.

At the very bottom, there are four steps listed, starting with giving notice to your employer asap.

But don't dilly-dally. Specifically, you have to notify your employer within 30 days of the incident causing the injury. Miss that deadline, and you're SOL.
Whether you handle it yourself, or go to a lawyer's office for an initial consult (hopefully a free initial consult), you need to get moving on this.
