Mostly it's trucked in and, while called 'fresh fish' that really means that it's just not been hard frozen. Here and there you may find somebody flying something special in via overnight but even then it's likely several days out of the water by the time that it gets to you. Not anything wrong with that but not really the same thing as actual real fresh fish.
Going to the DC fish market doesn't buy you anything. Virtually all of that is trucked in and just sold in a setting near the water. There's no fishing fleet around here to speak of for it to have come from otherwise. Nothing at any grocery stores around here will be truly fresh. Most all will have been flash frozen and thawed.
As they say, if you want fresh fish, then catch it yourself.

If it smells "fishy" then it's not fresh.
Seafood should have little to no fishy smell other than maybe very light.
Assuming OP is serious (always a chance, right?): We've given up on everywhere except Wegman's and Whole Foods. Quality elsewhere just isn't there. And no, it ain't cheap.
